Since the early 1920s the Swans have competed in numerous different leagues and associations.  They won premierships in the Swan Reach Football Association in 1921, and the Bairnsdale District Football League in 1956.  However, probably their most memorable achievement was their victory over Bruthen in the 1999 Omeo Football League grand final.  Swan Reach won by the utterly astonishing margin for a grand final of 125 points, 25.15 (165) to 5.10 (40).  When the same two sides again clashed on grand final day in 2003, however, it was a different story as the Bulldogs achieved revenge to the tune of 15 points.

After finishing second from last in both 2005 and 2006 the Swans recovered in 2007 to the extent of qualifying for their first grand final in four years.  However, the recently merged club of Omeo-Benambra proved too strong, and won convincingly by 70 points.

Two years later the Swans again qualified for the grand final and this time they made no mistake, overcoming arch rivals Swifts Creek by 36 points in an old-fashioned 'shoot-out'. Final scores were Swan Reach 25.19 (169) defeated Swifts Creek 21.7 (133).
